We adapted the NY/NJ Area Common Reporting Form and the NY/NJ Area Common Application Form for grantees to use to prepare final reports. If several funders have given your organization support for a particular project, only minor changes in the report information will need to be made for each funder.
Download the reporting form and answer questions in the order listed. Save your changes as a Word document. Your finished report should be emailed to firstname.lastname@example.org. If you do not have access to email, please feel free to send by regular mail. Please contact your program officer if you have questions.
